According to a nin.com post by Rob Sheridan, NIN played "31 Ghosts IV" live in Seattle on July 26, 2008. It then become a staple on the Lights In The Sky Tour.
It was played as a one-off in Japan on February 26, 2014 - the only time a song from Ghosts I-IV has been played live outside of the Lights in the Sky Tour.

It should be noted that much of the artwork embedded in the files of the official download of Ghosts I–IV from nin.com are not just cropped versions of the photographs in the PDF. In this case, the two photographs are completely different.
