This is the original album version. The track opens with programmed drums and a bouncy bassline, which establish a groove that lends it a somewhat disco feel. Reznor's vocals are largely spoken, which some have likened to the vocal delivery style of Talking Heads' David Byrne. Much like on "[[Head Like A Hole (song)|Head Like A Hole]]", the first section of each chorus is shouted. The drum machine used is the same as was used in the [[How To Destroy Angels]] track "[[Fur-Lined]]". Per the titles of the song's multitracks, a Bösendorfer piano was used on this song.

"Only" made its live debut on the [[2005/09/19_Phoenix,_AZ|third show]] of the [[With Teeth Fall Arena Tour]], and quickly became a setlist staple. This would continue for multiple tours, with "Only" appearing at most shows on the [[Performance 2007]] tour and almost every performance on the [[Lights In The Sky Tour]].
"Only" was not performed on the [[Wave Goodbye Tour]], and would make sporadic appearances in the [[NIN 2013-2014|2013-2014]] and [[NIN 2017-2018|2017-2018 touring cycles]].
During the [[Live: With Teeth]] and Performance 2007 touring cycles, a unique intro would be performed with [[Aaron North]] and Trent Reznor on guitars. During the [[Lights In The Sky Tour]], a different, longer intro was performed solo by [[Robin Finck]]. From the [[NIN 2013 Festival Tour]] onwards, the intro was performed the same as the studio version. The song was played with live drums until 2014, when [[Ilan Rubin ]] began playing bass while the band played to a backing track. It was then occasionally performed as a remixed/mash-up version in collaboration with [[Boys Noize]] on Act III of the [[Peel It Back Tour]], using Boys Noize's song "Loaded" as the base.
