Trent Reznor has stated that the song was not originally intended to the be on the album, but was rather meant for b-side material, due to not fitting in lyrically with the rest of the songs.
The song is notable for its instantly recognizable "hammer" synth bassline, similar to the one used in "Kinda I Want To"." The song also references "Down In It" in its use of the same recognizable melodic synth voice. An early version of the song had the title "I Can Make Myself Forget".

That's What I Get (Demo)
A clip of a purported demo surfaced on the internet in 2025. It is markedly different from both the album version and the Purest Feeling version. It has a completely different vocal melody and more of a pop music style. This may be the early version referred to as "I Can Make Myself Forget".
A second clip also surfaced, which seems to be taken from a different demo, one that sounds more similar to the version from Purest Feeling.
That's What I Get (Purest Feeling)
Taking a very different tone from its final version, the song is much less synthetic, with what at least sound like live drums and a synthetic saxophone backing, with the synthetic melody of the song being much more apparent. There is also a synth solo after the bridge verse that wasn't included in the final and some sampling that was omitted as well. The song also includes an extended, panning intro and extra lyrics and improvisation by Reznor towards the end of the song.
Live
"That's What I Get" was played live in its demo form during the VIVIsectVI Tour. It was then performed in a manner close to its album form throughout the rest of the Pretty Hate Machine Tour. It was retired from live play for many years at the conclusion of that tour. It was revived as a stripped-down version for the Peel It Back Tour in 2025. The song would start with Reznor alone on a digital piano, but would add atmospherics, bass, and beats as Alessandro Cortini and Atticus Ross joined Reznor on the B-stage.
Lyrics
But it meant everything to me
The first line of the second last verse was changed in the final recording to be How can you turn me into this The third line from the bridge of the song actually says "is changed to Maybe didn't mean that much" instead of "so much".
In its demo early form, the chorus lyrics are extended to say: "That's what I get (for trusting you)".
